Job description: Personal Assistant Service Caregiver

Job Summary
The CareFor PAS provides clients with assistance and companionship while recognizing clients' individual needs and encourages independence.

This job description represents an overview of the responsibilities for the above-referenced position. It is not intended to represent a comprehensive list of responsibilities. An employee should perform all duties as assigned by CareFor staffing team and according to clients' Individualized Service Plans and within the scope of a PAS license.

Essential Functions

  1. Assists clients with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual service plan. Allows and encourages clients to do as much of their own care as possible.
  2. Serves meals to clients in their homes. May assist in preparing meals following pre-planned menus. Encourages clients to eat a proper diet and take adequate fluids while respecting the clients' right to free choice. Records and reports changes in clients' eating habits to the CareFor staffing team.
  3. Initiates and participates in leisure activities provided for clients as described in Individualized Service Plan. Encourages clients to socialize and participate in planned activities or programs according to their Individualized Service Plan.
  4. Maintains clients' records daily in a timely manner and in accordance with company policy and procedures. This includes documentation related to: medication reminders as applicable; leisure activities; incidents and observations; reporting any changes in clients' physical condition and/or behavior; visitors; etc. Observes and reports the health and emotional condition of each client and promptly reports all changes to the supervisor.   5. May assist clients with medication as defined in medication reminder procedure. Assists or supervises clients who self-administer medication. Exhibits understanding of and follows medication policies and procedures. (Refer to state specific regulations regarding medication reminders).
  6. Maintains a clean, safe, and orderly environment for the clients. Performs general housekeeping following cleaning schedules for clients' laundry, bedrooms, dining area, living space, bathrooms, kitchen, etc.
  7. Follows proper procedures in emergency situations and responds promptly and positively to clients' requests for assistance - including emergency pendants, telephone calls, and requests from family and friends.
  8. Knows location of each client they are assigned to and if working in Senior Living community's signs in and out at the front desk.

Education and Experience
High school diploma or general education degree (GED); and twelve months related experience preferred and/or training in direct service with older adults, C.N.A., or equivalent combination of education and experience.

Knowledge of dementia particularly of the Alzheimer's type is highly preferred.

Certifications, Licenses, and other Special Requirements
Certified Nursing Assistant Certification or Home Health Aide Certification preferred.

In accordance with state law, may need to possess current state certification and follow regulations to maintain current certification.

Every other weekend if applicable for scheduling, weekend, evening or night work if applicable to ensure shift coverage

Management/Decision Making
Makes standard and routine decisions based on detailed guidelines with little independent judgment. Problems are generally solved with clear, detailed guidelines or by reporting them to the CareFor staffing team.

Knowledge and Skills
Has a basic level of a skill and can follow written or verbal instructions. Has basic technical skills to complete a few repetitive and well-defined duties. Has a basic knowledge of the company in order to answer basic questions. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to communicate effectively in English with clients, family members and other caregivers of the company. Ability to apply common sense understanding to carry out instructions furnished in written or oral form. Uses good judgment in emergency situations, and seeks assistance when needed.

Duties include:

  • Adhering to client's rights
  • Maintaining client confidentiality and adhering to HIPAA regulations
  • Providing for and assisting with personal care needs, such as bathing, dressing, grooming, hair care, shaving, oral care, and perineal care
  • Cleaning and filing fingernails
  • Assisting with toileting using bedside commode, bedpan, and urinals
  • Assisting with ambulation including the use of walkers, canes, and wheelchairs as needed
  • Performing home management functions such as light housekeeping of areas of the residence that the client uses, laundry, bed making and cleaning
  • Planning meals, shopping for groceries, preparing and serving food, feeding, and clean-up
  • Responsible for referring to the client's Individualized Service Plan regarding reminding and assisting with client's normally self-administered medications and medication reminders.
  • Reporting changes in the client's condition or family situation to the CareFor staffing team within 24 hours
  • Responding to and reporting emergency situations to the CareFor staffing team
  • Documenting services provided on the appropriate flow sheet and notes in CareFor PAS binder.
  • Accompanying client to schedule appointments
  • Participating in in-service programs, monthly in-service required per state regulations.
  • Submitting documentation within required time frames
  • Caring for intact skin to prevent skin breakdown
  • Assisting with transfers to assure client safety
  • Caring for Alzheimer's patients
  • Assisting with proper body alignment to prevent complications from immobility
  • Transporting clients or assisting with errands
  • Assisting with turning on oxygen concentrator only as directed by the client
  • Monitoring and recording client intake and output
  • Assisting with ROM or prescribed exercise program
  • Assisting with deep breathing and coughing exercises
  • Assisting with external urinary drainage
  • Perform other duties as assigned consistent with the terms of this job description and related to client's independent living where the client cannot perform such duties due to the client's health or age limitations.
